Formed in 2001 as subgroup of the Perinatal Outreach and
Education Task Force in response to incidence of low birth
weight and infant mortality consistently higher for the black
population than for the rest of the population. Meets the
second Thursday of the month three times per year to devise
and implement strategies to improve black infant birth outcomes. |

This is an active network of advocates promoting and supporting
breastfeeding through different activities including education
of families, our community and health care providers.
The Coalition meets 7:30 a.m. every two months on the third
Thursday at Denny’s, 1525 McHenry Ave. |

The HEART Coalition was convened in 1999 due to the high
rates of heart disease in Stanislaus County. The coalition’s
mission is to reduce cardiovascular disease risk factors to
improve the health status and quality of life of the residents
of Stanislaus County. They continue to increase awareness
through their website at www.heartcoalition.org,
the creation of a Cardiovascular Resource Guide, and by conducting
educational activities such as a Healthy Heart Info Fair and
Heart Calendar. |

The Hepatitis C Task Force is representeded by members of
the HIV/STD program, Heath Service Agency Epidemiologist,
Public Health Officer, Behavior Health and Recovery Services
Alcohol and Drug Program, Veterans Services, Jail Medical
(CFMGJ), Kaiser, Communicable Disease Program, Golden Valley
Homeless Program and non-profit agencies. The primary task
of this group is to develop a Hepatitis C plan for Stanislaus
County. |

The primary task of the Local Implementation Group is to
develop a comprehensive HIV plan that is based on scientific
evidence and community values and is responsive to the local
epidemic. The group meets bi-monthly and is represented by
the affected and infected population, behavioral and social
scientists, an epidimiologist, HIV/AIDS prevention service
providers, health department staff, representative of healthcare
delivery systems that serve HIV infection, and community advocates. |

A group of private and public organizations, agencies,
business leaders and concerned citizens working together
to protect infants and young children against vaccine preventable
diseases.
DMC Foundation is the lead agency. Meetings are held
the 4th Thursday of every month at noon in the Martin
Conference Room at Stanislaus County Health
Services Agency. We strive to reach
the Healthy People 2010 goal of 90% immunization coverage.
With special projects and education, the coalition promotes
awareness on the importance of timely childhood immunizations.
Current activities include: annual Immunization Administration
Techniques for Medical Assistants, annual RIDE provider
appreciation baskets and annual National Infant Immunization
Week and California Toddler Immunization Month activities

This coalition is made up of a variety of organizations
and individuals that come together to discuss, collaborate,
and work on projects that are related to the promotion of
healthy eating and physical activity. |

this committee holds at least two annual meetings, has
representatives from education, the dental profession, parent
groups, school nurses, and others interested in dental disease
prevention in children. It's purpose is to assist in developing
and reviewing the project application for funding, and to
have a yearly project that will increase dental disease prevention
awareness for children in our county. |

A multidisciplinary group that shares the goal of assisting
women to have healthy, fullterm infant births. The group meets
regularly and have developed strategies to reduce the incidence
of low birth weight babies born in our county. |

Strives to decrease the number of vehicle-related injuries
and fatalities in Stanislaus County. The Safe Communities
Coalition developed a strategic plan to meet this goal by
focusing on four priority areas: bicycles and pedestrians,
child passengers, teens and general roadway safety. Current
activities include coordinating Walk to School Day events
at select schools, bike helmet diversion programs and child
passenger safety training. |

The mission of the TOPS Coalition of Stanislaus County is to lead the community in the development and promotion of tobacco control outreach and prevention services.
Through the involvement of a wide spectrum of community members, with a collaborative knowledge and expertise of the entire community, the TOPS Coalition is able to advocate for and
assist in the development and implementation of comprehensive tobacco control efforts. |

TOPS Coalition Priorities:
• Continue to build and develop the TOPS Coalition to increase community involvement and
improve the reach and effectiveness of tobacco control outreach and prevention services.
• Coordinate smoking cessation programs in the community and through local worksite
wellness programs.
• Support the continued success of PHAST (Protecting Health And Slamming Tobacco). This
countywide youth coalition is a partnership program between the Tobacco Education
Program and the Stanislaus County Office of Education TUPE program.
• Partner with other agencies to improve compliance of statewide and local smoke-free
indoor workplace laws, specifically in bars and restaurants.
• Continue to advocate for and implement retail licensing in at least two jurisdictions within
Stanislaus County.
• Serve and educate the community through active participation in public discussions on
tobacco control and health, including those found in print media, letters to the editor,
community blogs, or local presentations.
