Adolescent Family Life Program/Cal Learn [AFLP] |
| Adolescent Family Life Program (AFLP) and Cal Learn are home visiting
programs for pregnant and parenting teens that promote healthy births,
improve life skills, delay repeat pregnancy, and encourage school attendance and completion.
Services include in home case management by public health nurses or community
health workers with assessment, guidance, referrals and assistance in accessing
support services. Services are available to teens up through age 19 year.
Target Population:
AFLP-Open to any pregnant or parenting teen residing in
Stanislaus. Cal Learn-Any pregnant or parenting teen who receive TANF. |

| Services are funded by State and federal grants
and Medi-Cal. No client is denied service due
to inability to pay. |
